High Fidelity Wraparound Facilitator

High Fidelity Wraparound has a position opening for a full time, temporary Wraparound Facilitator. This is for a 1 year maternity leave starting September 2019. 

Wraparound refers to a planning process by which families and children and youth with complex needs are empowered to work collaboratively with formal and informal supports to develop a plan that will help the family realize their vision for a better life. A facilitator engages and partners with the family/youth/child over the course of the Wraparound process.

 Duties and Responsibilities:

  • Provide leadership in the assessment, development, implementation and evaluation of individualized wraparound plans
  • Assume responsibility for the coordination and facilitation of child and family teams
  • Consult and cooperate with community systems to facilitate linkage, advocacy and follow up with the focus on attaining goals
  • Assess the strengths and needs of families, facilitate the development of an individualized child and/or family plan and monitor the progress in meeting established goals
  • Provide crisis management for persons served; make linkages as appropriate
  • Provide follow-up to the team to ensure continued success after wraparound services
  • Advocate for families and children in schools, communities, courts and social service agencies as needed
  • Assist each family, as appropriate, in meeting their basic needs for housing, food, transportation, employment, day care, social support networks, etc.
  • Arrange for adjunctive services or therapies as needed
  • Maintain person served and program records in accordance with applicable standards
  • Provide services to family members in accordance with the policies and procedures of Hull Services and the standards established by the Alberta Association of Services for Children and Families; ensure humane, individualized treatment that is planned and sensitive to diversity and designed to foster the strengths of each family
  • Participate in the evaluation processes and procedures and actively promote quality standards for the profession.
  • Participate in a close consultation relationship with the Program Coordinator and work within the policies and procedures outlined for the program
  • In general, behave in ways that promote cooperation and harmony and foster a positive, optimistic environment for children, families, colleagues, and others

Skills and Qualifications: 

  • Degree or diploma in Human Services or Social Sciences
  • Relevant experience in working with youths and their families and knowledge of crisis intervention, communication skills and family education
  • Perceived ability to model appropriate behaviours and exercise good clinical judgment in working with distressed individuals and families
  • Clear Police Check including Vulnerable Sector Check; Child Intervention Record Check
  • Must have a valid driver's license, reliable transportation and adequate automobile liability insurance
