What we do and why we do it:

Pathways to Housing is a recognized innovative, best-practice service delivery model for ending homelessness. This model seeks to end homelessness by providing ‘Housing First’ coupled with harm reduction services provided by a multidisciplinary Assertive Community Treatment (ACT) team. This program serves homeless individuals with mental health, physical health and addiction disorders. Our multidisciplinary team provides our clients with housing and follows ACT principles in supporting them toward independent living.

The Community Integration Specialist is an integral part of the Assertive Community Treatment team. They support clients to maintain housing in community settings and assist with communication and advocacy to achieve and maintain community integration and self-sufficiency. This support includes working with an interdisciplinary team to assess the needs of each individual and then to provide appropriate interventions in community settings.  This position reports to the Clinical Team Lead. 

What you will do: 

  • Responsible for working with the program clients using principles of psychiatric rehabilitation and multiple behavioural management strategies to assist clients to move toward independent living in the community
  • Working as a liaison to establish and maintain professional relationships with external and internal stakeholders
  • Using principles of Housing First and harm reduction, along with knowledge of the Residential Tenancy Act to assist in client advocacy and system navigation
  • Working within the context of client-centered care, evidence-based practice, and multidisciplinary service delivery, the Community Integration Specialist provides housing focused interventions to assist clients to achieve community integration and self-sufficiency 

What you will bring: 

  • A bachelor’s degree in a relevant social science, humanities, health, or rehabilitation discipline
  • Minimum two years’ experience working with clients facing chronic homelessness
  • A high level of flexibility required
  • Ability to work collaboratively in a multidisciplinary team model and able to work effectively within an innovative outreach service-delivery model
  • Previous experience working within the homeless sector, or closely aligned sector, an asset
  • Experience with community outreach and crisis intervention
  • Demonstrated effective communication, organization, advocacy, and rapport-building skills
  • Knowledge of the impact of mental illness and substance use on housing outcomes
  • Familiarity with social, health and non-profit services in Calgary
  • Knowledge of Motivational Interviewing an asset
  • Ability to multi-task in a fast-paced environment
  • Class 5 non-GDL drivers license, a personal vehicle, and $2 million liability insurance are required
  • Current First Aid / CPR required
  • Must take on-call shifts as per rotation
