Case Manager

We are seeking a Case Manager for our Shift Program. Program services are delivered from a harm reduction and rights based approach to sex work, providing services to individuals across the continuum of choice in their sex work involvement.

The Case Manager, Shift is responsible for providing comprehensive case management services to adults of all genders who have worked or are currently working in the sex industry. The focus of service delivery is to enhance safety and well-being through case management and outreach activities for those who are actively working in the sex industry along with supports to leave the sex industry if this is a client identified goal. Case management activities include assessment, crisis intervention, supportive counselling, community based outreach, development of service plans, distribution of harm reduction supplies and assistance with basic needs, advocacy and referrals. Client support services are available both over the phone and in person.

This is a full-time position (35 hours a week) working Monday to Friday.  A flexible schedule and ability to work evenings and weekends is required. Some inner-city travel may be necessary. This position will be located in the Calgary office and will report directly to the Team Lead, Support Services.

Key Accountabilities

Case Management

  • Assesses client needs in the areas of health care, housing, income security, social supports, legal, substance use and/or addiction issues and harm reduction
  • Engages clients in the development of service plans based on client-identified needs (including setting measurable client driven goals, identifying associated tasks and establishing attainable timelines)
  • Provides supportive counseling, crisis intervention, and case management both in the office and in community based settings
  • Ensures on-going support to clients through the development of mutually agreed upon contact plans, maintaining regular appointments and telephone calls
  • Coordinates a network of services on behalf of clients by bridging communication gaps between clients and service providers, attending and/or arranging case conferences and accompanying clients to appointments as appropriate
  • Provides referrals to other HIVCL services and/or other community services in relation to health care, housing, income and social supports, basic needs, etc.
  • Advocates on behalf of clients to address barriers to accessing services
  • Distributes of harm reduction supplies as required based on client needs

Outreach & Community Engagement

  • Provides community, mobile and street based outreach as required on a weekly or bi-weekly basis as per established program requirements (e.g. participation in street-level outreach activities with HIV Support Services Case Manager, participate in community based outreach activities with other service providers such as Safeworks)
  • Prepares and distributes population specific harm reduction tools and safety information
  • Collects “Bad Date” information, manage and distribute “Bad Date” sheet to community and sex worker contacts
  • Provides online outreach services to diverse sex worker communities
  • In collaboration with the Team Lead, develop and maintain inter-agency collaborative relationships with community partners to enhance service delivery and outreach to program target populations

Support & Administration

  • Participates in and support program activities facilitated within the drop-in space such as food programming and client engagement activities
  • Ensures program accountabilities through maintaining program statistics and service delivery outcome information as per database requirements
  • Participates in ongoing program evaluation and reporting processes as required
  • Continuously monitors quality improvement and evaluation of client files and provides information on potential areas of development
  • With the support of the Team Lead, facilitates case discussions to support knowledge development and contribute to case consultations
  • Participates as a collaborative and supportive team member of the Support Services team

Qualifications/ Key Competencies

  • Degree in Human Services field with two (2) years of direct work experience in a frontline client-focused support setting is preferred
  • Equivalent combinations of experience and education will be considered
  • When applicable, registration with appropriate professional body must have active registration or qualify for registration (e.g. ACSW)
  • Demonstrated proficiency in client engagement, case management and supportive counseling
  • Demonstrated ability in case management activities including assessment, service planning, crisis intervention, referrals and outreach
  • Foundational knowledge of, and a theoretical orientation to, harm reduction, trauma informed care, intersectionality, anti-oppressive and sex workers’ rights practice frameworks
  • Strong knowledge of mental health, substance use/addiction issues and interventions
  • Good working knowledge of local social service programs and community resources
  • Demonstrated cultural awareness and responsiveness in working across diverse populations
  • High ethical standards and professionalism with a demonstrated ability to maintain confidentiality
  • Effective oral and written communication skills
  • Proven ability to plan, organize and prioritize competing tasks and timelines
  • Demonstrated analytical and critical thinking skills with the ability to problem solve
  • Knowledge of HIV, gender diversity, sex work and related issues is desired
  • Current Non-Violent Crisis Intervention (NVCI), Suicide Intervention (ASIST) and First Aid/CPR certifications preferred
  • Provision of a criminal record check, including a vulnerable sector search, required
  • Valid driver’s license and access to reliable transportation is required
