We are currently seeking someone who has a passion for working with youth aged 15-24 years old. The Transitions Worker will be responsible for supporting youth with community resources, family finding and natural supports, navigating the justice system, health and well-being, school and employment, pregnancy, child birth and parenting, safe and affordable housing, addressing various complex needs including addiction and mental health and teaching Life Skills that will allow the youth to transition successfully into independence. A vehicle is necessary.

The successful candidate will have:

  • Diploma/Degree in Human Services
  • Knowledge and experience working with the justice system
  • Knowledge of appropriate community resources
  • Family Finding training would be an asset
  • Police check and Child Intervention Record Check (current within six months)
  • Strong writing, organizational, program planning and interpersonal skills
  • Valid Driver’s license and adequate insurance coverage
  • Own transportation

Salary Range:     $45,545.00 to $50,270.00
