Social Worker

 Salary based on previous experience and RSW status.

POSITION RESPONSIBILITIES INCLUDE: 1. Promote the philosophy, mission objectives and policies of the Foothills Country Hospice Society (FCHS), while interacting with residents, loved ones, employees and the community. 2. Psychosocial assessment includes: genogram, history of significant life experiences, clarify information the resident has or would like to know related to their illness, inquire about life goals and create a plan to meet unmet goals. 3. Work with the resident and members of their social network to deal with psychosocial stressors such as anxiety, guilt, fear or sadness through counselling, education or short term psychological techniques. 4. Assist with family meetings, case conferences, team conferences to address complex needs or issues. 5. Formalize and implement advance care planning documents: personal directives, enduring power of attorney, wills and end of life wishes. 6. Provide resources to address the following: finances, forms (pre and post death), end of life services, legal contracts and private practice rehab practitioners. 7. Chart in progress notes. 8. Assist families / significant others with the grief process by participating in bereavement follow up programs. 9. Collaborates and liaise with other professional disciplines (community physicians, agencies and clergy) to enhance the continuity of care to residents, families, volunteers and employees. 10. Attend weekly rounds. 11. Complete monthly / quarterly reports for Executive Director. 12. Implements, develops and revises social work policies and procedures in consultation with the Clinical Team and Executive Director. Preferred Minimum Requirements • Valid registration with Alberta College of Social Work • Palliative Care experience preferred • Minimum 3-5 years of clinical experience • Experience with program development • Experience with grief and bereavement support • Demonstrate excellent communication and organizational skills • Demonstrates ability to work independently with minimal supervision. Accountability • Hours of work: .5 FTE – 20 hours per week (must be available days, evenings and some weekends) • Three (3) additional hours per month allotted for Hospice led Grief Support Group • Reports to Executive Director • No supervisory responsibilities • Job posting closes April 6th, 2018
