Hospice Calgary guides children, teens and adults through the grieving process as they face a life threatening illness and death of a loved one. Our team of professionals provides individual, family and group support as well as 24 hour end-of-life care. We operate two centres of care: Sage Centre for counselling and education services and Rosedale Hospice for residential end-of-life care.

Position Summary:

Responsible for facilitating the weekly Living with Cancer Program, clinical assessment/monitoring of the clients, and the provision of psychological and spiritual support. The position builds and strengthens a network of community professionals and volunteers to meet the evolving interests and needs of program participants while enhancing program resources. The position also contributes to an ethical, safe, and inclusive environment through knowledge and practice of the organization’s policies and procedures.  

Primary Duties and Responsibilities

Client Assessment and Support

  • Assesses and intakes  new clients and family members into the program
  • Provides information to clients and family members about Hospice Calgary, advance care planning, hospice access, and availability of resources
  • Does home, hospice, and hospital visits with clients and families as well as provides regular telephone support
  • Ensures supports are in place to meet the needs of the clients
  • Acts as a strong advocate for clients who are navigating the health care system

Program Delivery

  • Facilitates the weekly Living With Cancer Program  
  • Responsible for  ongoing evaluation and development of the program
  • Coordinates provision of internal and community professional resources for program delivery
  • Supports participants and family members with current physical, emotional, spiritual and psycho-social  needs
  • Works with the Volunteer Manager and the Director of Community Hospice Services to provide training, supervision, and mentoring to the program volunteers
  • Writes and revises program materials as needed
  • Uses professional judgement to ensure the integrity of the service is maintained
  • Collaborates with the Community Hospice Services team, the Medical Director, Spiritual Care Coordinator, and Rosedale Hospice to provide seamless support to participants and exploration of client issues to determine the best course of action

Administrative Duties

  • Provides professional consultation to other agencies and organizations in the community
  • Participates in team and supervision meetings as required
  • Ensures regular documentation on all program participants, and keeps statistics on program attendance, offsite visits, community networking, etc.
  • Continuously researches best-practice and keeps current information available for clients and colleagues


  • Must be a Registered Nurse with current CARNA registration
  • Minimum of 2 years palliative care experience
  • General computer skills
  • Group management and problem solving skills
  • Knowledge of, and commitment to, the philosophy of hospice care, with particular regard to the needs of patients with advanced cancer diagnoses
  • Strong written and verbal communication skills
  • Valid Alberta Class 5 driver’s license and own transportation required
