We are currently recruiting for: MIDDLE SCHOOL FAMILY CONNECTOR (HR116)

0.8 FTE Temporary Position (1 year, 28 hours/week)

Recruiting salary: $33,752.00 - $41,366.00 annually

Position Summary:

The primary goal of the Family Connector is to assist, and build the capacity of, select South East/North East Middle Schools by connecting students and their families to community resources through referral and outreach services.



  • Provide support by taking referrals from school and agency staff to work with individuals and families over the phone and/or in the client’s school, home or community to facilitate referrals and provide assistance in accessing services
  • Provide emotional support and share information as needed
  • Assist schools requiring external resources to deal with critical incidents
  • Establish and maintain strong working relationships
  • Contact, and establish relationships with, various service providers in the community to ensure accurate knowledge regarding programs and services throughout the city
  • Through consultation, education and resourcing, support the ongoing capacity of school administration to foster resiliency of children in school and community
  • Establish and maintain client files and other agency required record keeping accurately, in accordance with ethical standards of practice and in a timely manner
  • Ensure the accurate completion of all evaluative tools utilized in the program
  • Assist in the development of program planning and coordination
  • Assist with other agency projects as they arise
  • Follow ethical guidelines as outlined by the incumbent’s professional association,  or the ACSW Code of Ethics if not a member of any professional association.

Qualifications and Experience:

  • An undergraduate degree in Social Work, Social Sciences, Child & Youth Care or a related field
  • Excellent written and oral communication skills
  • A commitment to working from a collaborative stance with family, school and community systems
  • Assessment skills, knowledge of community resources and computer literacy
  • An ability to work with diverse populations

The individual must successfully complete all required security clearances.
