Family Support Worker

Family Link Consultant

Accountable to:

Executive Director

Summary:

The Family Link Consultant will be responsible for the intake and provision of direct services to families, agencies, and professionals seeking information from The Children’s Link Society. All duties will be carried out in a manner consistent with the Mission Statement and the Policies and Procedures of the Children’s Link Society.

16/hrs week Tuesday and Friday. Hours require flexibility as dictated by the needs of the organization and the families it supports.

General Team Expectations

  • Ensure that all families, agencies, and professionals have access to needed information;
  • Promote communication and information sharing between service providers and community agencies;
  • Collaborate within the disabilities community;
  • Attend team meetings and scheduled training sessions;
  • Ensure team members are aware of all current and any new information/resources;
  • Communicate appropriately and effectively with all team members;
  • Respect and follow office procedures and protocol;
  • Effectively utilize of all resources critical to the operation of The Children’s Link Society;
  • Ensure confidentiality of all family and personal information;
  • Identify training opportunities for personal and professional growth;

Key Accountabilities:

Completion of referrals 

  • Intake of referrals;
  • Complete necessary documentation;
  • Utilize effective listening and communication skills to assist families and others in identifying needs for information and support;
  • Gather current and accurate information and do necessary research;
  • Ensure referrals are reviewed, prioritised, and completed appropriately;
  • Identify complex referrals to team and/or Executive Director;
  • Meet with families as required;
  • Complete follow up procedure with identified referrals;
  • Input completed referrals into database.

Additional Responsibilities 

  • Refer families to Family Link program by completing necessary documentation
  • Provide changes in agency/program contact information to appropriate team members to ensure that templates, information lists, etc. are updated.
  • Seek appropriate consultations (internal and external) to support families appropriately.
  • Complete and submit timesheet and nominal roles within 5 business days of the end of the month.
  • General telephone answering, communication and administrative back-up functions
  • Attend and assist with resource fairs and coffee socials as required to support and promote the agency

Requirements:

  • Personal experience with a child with special needs
  • Familiar with FSCD
  • An appreciation of issues relevant to diversity
  • Professional/volunteer experience supporting families who have a child(ren) with special needs would be an asset
  • Issues, service systems and supports in the Region 3 Calgary and Area relating to children with special needs and their families
  • Strong written communication skills
