Family Facilitator



FAMILY FACILITATOR POSITION- 13th month Temporary (Maternity Leave)

The Patch Program has a Full-Time, Temporary Family Facilitator posting available. This role carries a flexible 40 hour/week work schedule, which may include some evening and weekend work.

Patch is a neighbourhood Hub that provides a wide range of integrated services to children, families and their community in Calgary Housing Company complexes and surrounding neighbourhoods in Calgary. The central goals of the Hubs are to decrease poverty, increase health and wellness of children and families, and build thriving communities.

The Family Facilitator position will work toward the goal of helping vulnerable families increase their sense of personal and family wellbeing.   The Family Facilitator will support families in developing effective “positive parenting” skills, which will include providing education around healthy infant/toddler/child/youth development.  Within the continuum of Patch services, the successful candidates will have the opportunity to participate in program development, along with providing a variety of support services to families. This position requires an individual who is flexible, proactive and goal directed.

Duties and Responsibilities Include:

  • Providing in-home support services to families including, persons served skill building, crisis management, persons served advocacy and community resourcing
  • Assessing needs, supporting families in prioritizing needs and creating Wellness Plans
  • Providing facilitated self-help and parenting skills training to persons served
  • Coordinate and co-facilitate skill based groups using the Nurturing Parenting Curriculum
  • Developing safety plans influenced by the phases and activities of High Fidelity Wraparound practice
  • Creating Developmental Support Plans using the Hand in Hand Infant Wellness Model
  • Life skill enrichment using Skills for Psychological Recovery non-disaster Model.
  • Systems navigation support,
  • Keeping thorough and up-to-date records of person served contact and the services provided. This includes all relevant program statistics for funders and for evaluation and outcomes purposes
  • Liaise with all members of the Patch team as well as with community partners to best meet the needs of the families served.

Skills and Qualifications: 

  • Degree or diploma in human services or social sciences.
  • Experience working with families in a community based setting.
  • Proficient use of core counselling skills (e.g. able to accurately understand and address the needs of Person’s Served while maintaining professional boundaries).
  • Familiarity with computer programs, databases and evaluation systems.
  • Experience with facilitating skills based groups and program development are definite assets.
  • Ability to speak a second language is an asset.
  • Interest in working in an environment that is fun and encouraging of self-exploration and professional growth.
  • Ability to contribute positively to team moral. 
  • Strong awareness of, and ability to manage, ones own emotions.
  • Ability to work independently and as a team member.
  • Ability to work evenings and/or weekends as needed.
  • Excellent problem solving, communication, capacity building, and interpersonal skills.
  • Willingness to use own vehicle to travel to home visits.
  • Valid Drivers License and reliable transportation.
