The Alex Community Health Centre is a not-for-profit organization that has been operating in Calgary since 1973. The Alex provides services and programs to individuals and families who experience barriers to health. Using a multidisciplinary team approach, we focus care on assessment, intervention and promotion of all aspects of health. We embrace a model of healthcare that is accessible, responsive and participatory. In addition to its two Health Centers, we run a Youth Centre, two Mobile Community Health Buses, one Community Dental Bus, four Housing Programs, a Breast Feeding Clinic, a Community Food Centre and a Bottles4Health Program.

Pathways to Housing is a recognized innovative, best-practice service delivery model for ending homelessness. This model seeks to end homelessness by providing ‘Housing First’ coupled with harm reduction services provided by a multidisciplinary Assertive Community Treatment (ACT) team. This program serves homeless individuals with mental health, physical health and addiction disorders. Our multidisciplinary team provides our clients with housing and follows ACT principles in supporting them toward independent living.

The scope of work and responsibilities include, but are not limited to the following:

  • Responsible for working with the program clients using principles of psychiatric rehabilitation and multiple behavioural management strategies to assist clients to move toward independent living in the community
  • Working as a liaison to establish and maintain professional relationships with external and internal stakeholders
  • Using principles of Housing First and harm reduction, along with knowledge of the Residential Tenancy Act to assist and assess client moves
  • Working within the context of client-centered care, evidence-based practice, and multidisciplinary service delivery, the Community Integration Specialist provides housing focused interventions to assist clients to achieve community integration and self-sufficiency

Desired qualifications include:                                                                 

  • A bachelor’s degree in a relevant social science, humanities, health, or rehabilitation discipline
  • Minimum two years’ experience working with clients facing chronic homelessness
  • A high level of flexibility required
  • Ability to work collaboratively in a multidisciplinary team model and able to work effectively within an innovative outreach service-delivery model
  • Previous experience working within the homeless sector, or closely aligned sector, an asset
  • Experience with community outreach and crisis intervention
  • Demonstrated effective communication, organization, advocacy, and rapport-building skills
  • Knowledge of the impact of mental illness and substance abuse on housing outcomes
  • Knowledge of social, health and non-profit services in Calgary
  • Knowledge of Motivational Interviewing an asset
  • Ability to multi-task in a fast-paced environment
  • Class 5 license and personal vehicle required
  • Current First Aid required
  • Must take on-call shifts as per rotation
