Since 1973, our cost-effective programs have provided preventative care in a welcoming, easy to access environment, moving people from poverty to stability and from crisis to wellness. Using a multi-disciplinary team approach, we focus care on assessment, intervention and promotion of all aspects of health. We embrace a model of health care that is accessible, responsive and participatory. In addition to the Family Health Centre, we run a Seniors Health Centre, a Youth Centre, two Mobile Community Health Buses, one Dental Health Bus, four Housing First Programs, a Community Food Centre and the Children’s Legal & Educational Resource Centre (CLERC).

Pathways to Housing is a recognized innovative, best-practice service delivery model for ending homelessness. This model seeks to end homelessness by providing ‘Housing First’ coupled with harm reduction services provided by a multidisciplinary Assertive Community Treatment (ACT) team. This program serves homeless individuals with mental health, physical health and addiction disorders. Our multidisciplinary team provides our clients with housing and follows ACT principles in supporting them toward independent living.

We are currently seeking a motivated, flexible, client-focused Clinical Team Lead for one of our ACT Teams.

The scope of work and responsibilities include, but are not limited to the following:

  • Actively participates as a member of the Pathways management team
  • Provides supervision, direction and support to the Clinical Team
  • Provides and delivers training to clinical staff utilizing the ACT model
  • Conducts client intakes and assessments
  • Second on call is a part of duties
  • Contributes towards ensuring that the program is sufficiently flexible and dynamic to meet the needs of clients  who have been chronically homeless and have had or continue to have involvement with the criminal justice system
  • Ensures that client self-identified goals for recovery; rehabilitation and community integration are supported by appropriate services and best practices in the mental health field 

Desired Qualifications Include:

  • Master’s Degree in a related field is preferred
  • Demonstrated Leadership in the homeless serving sector
  • Experience working in a clinical position an asset
  • Experience working in a management role an asset
  • Experience working with urban homeless populations with serious mental illness, substance addictions, justice involvement and complex health issues
  • Familiarity with the correctional system and resultant challenges clients face
  • Experience working with Evidence Based Practices
  • Experience working in an interdisciplinary team environment
  • Proven leadership, interpersonal communication and writing skills
  • Patience, creativity, flexibility, compassion and sensitivity to persons who have been chronically homeless and struggling with multiple barriers
  • Must have a full class 5 driver’s license
