Case Manager (1.0FTE)

Centre for Refugee Resilience

Division:                 Community Development and Integration Services        

Responsible to:     CDIS Manager and Centre for Refugee Resilience Team Lead

Functional Statement:

The Case Managers are a newly funded component of the Centre for Refugee Resilience. They will act as integral team members in building resilience with refugees and immigrants who have experienced trauma, through coordinated community, social, and mental health support.

 Primary Duties and Responsibilities include but are not limited to:

●      To coordinate the care delivered to a group of patients

●      To facilitate communications between care providers and external agencies

●      To oversee the follow-up of consultations and that they are linked to other services

●      To eliminate task and intervention duplication, to plan for care delivery, to implement           the plans of  care and to ensure their follow-up

●      Identifies the health needs of the patient and the expectations of the family

●      Makes an assessment of the resources and support networks available and assists the        patient and family to access needed services

●      Develops a care plan with the patient and other care providers

●      Coordinates all tests, examination, consultations and care according to the care plan

●      Seeks to ensure the continuity of care by avoiding fragmentation and duplication of       services

●      Evaluates the plan of care and achievement of outcomes and adjusts as necessary

●      Coordinates preparations for patient discharge including post-discharge requirements;         orienting and training family members and arranging appropriate follow up resources

●      Establishes communication with other team members, transmits the information            required to them and invites them to team meetings

●      Contacts the multidisciplinary team members involved, issues reminders and follows            up on the interventions to be implemented

●      Maintains relations among the team members and departments/agencies involved in      the follow-up

●      Facilitates the flow of information among the members of the multidisciplinary team

●      Ensures the plan of care is clearly documented

●      Coordinate and participate in case conferences as needed

●      Work collaboratively with a diverse and multi-disciplinary team within CCIS

●      Practice personal self-care and support the needs of team members

●      Complete evaluation and reporting of program activities that fit all funder outcomes and indicators as outlined by the Team Lead


●      Post-secondary degree/diploma in Social Work

●      Experience in case management

●      Demonstrated experience in working with culturally diverse individuals and families and in working with mental health

●      Ability to work collaboratively and communicate effectively within a multicultural and multidisciplinary setting

●      Knowledge and competency in cultural diversity and special issues relating to refugees

●      Fluency in Kurmanji or Arabic will be considered an asset

●      Ability to work a flexible schedule that includes occasional evenings to accommodate clients

●      Ability to take initiative, work independently, organize one’s work and records

●      Experience with not-for-profit agencies considered an asset

●      Experience in managing confidential client and program data

●      Competent verbal and written communication, computer, and facilitation skills

●      Ability to communicate in a second language considered an asset

●      Ability to plan, organize, implement, and evaluate activities

●      Ability to positively work with challenges

●      Knowledge of community and mental health resources in Calgary

●      Must have a valid driver’s license and own reliable transportation.

●      Must have current criminal record check.
